Forms and Letters

Here you will find any forms you need to fill out for registering your child at Kids Club.  Every year, you must re-register your child so that we have an updated version of contact details and information on your child/children.  You can, or course, collect a form from Kids Club.  However, if you'd prefer to print your own, feel free to do so, or email to us at [email protected]
Be advised, if we do not have a registration form, we cannot collect your child from school, or allow them to be involved at playschemes.
Each child will need a separate and current registration form.
All children are welcome to come to our holidays club, from all schools. 

Playscheme Booking Forms are sent out and uploaded TWO WEEKS before the holidays.  Please be patient and complete the correct booking form.

If any of your details change throughout the year, please let us know.  These details are our ways of contacting you to keep you informed of up coming playschemes, invoices, and contact details in case of emergency to do with your child.  If your details are out of date or wrong, then we may not be able to contact you.
